If you are ready to enroll, you have four different ways to sign up.

1. Enroll by mail or fax. Complete the South Carolina Health Connections Choices form in the enrollment packet and send it to Healthy Connections Choices. Call 1-877-552-4642 to get your forms.
2. Enroll over the phone by calling South Carolina Healthy Connections Choices toll free at 1-877-552-4642.
3. Enroll online at the South Carolina Healthy Connections Choices Web site at www.SCchoices.com
4. Enroll in person by meeting with a community enrollment counselor. Call 1-877-552-4642 to make an appointment.

Healthy Connections Choices is a new program that helps Medicaid members enroll in the health plan of their choice.
